IPHA 75th Annual Meeting & Illinois Oral Health Conference: Public Health on the Cutting Edge

2016 IPHA Call for Award Nominations


The Awards Committee for the 75th Annual Meeting is requesting membership nominations for this year’s awards.  The awards will be presented during the Annual Meeting at the Friends of Public Health Dinner to be held September 14, 2016, 5:30pm - 8:00pm at the Wyndham City Centre in Springfield, Illinois.

This is your invitation to recognize your fellow workers for their contributions in the public health community.  Individuals or groups involved in any aspect of community, public health and human services should be nominated for these leadership awards.

Please review the criteria for each award and select the category most suited to your nominee’s accomplishments.  You are not limited in the number of nominations you submit; however, you must be a current IPHA member to initiate a nomination. 

All nominations must be submitted online at www.ipha.com and must answer the following:

  1. Describe the outstanding contribution of major significance to public health which makes the nominee eligible for the award chosen.
  2. Describe the impact of their contribution.
  3. Why should this nominee be considered for the award?

The deadline for submission is 5:00pm on August 15, 2016.

Candidates for this award shall be retired and shall have been association members in good standing for 20 years.  Retired members with fewer than 20 years of membership in the association may be considered by the committee and recommended to the Executive Council for this award. 

This award is given to an individual whose dedication to volunteer efforts to improve the public’s health is exemplified through a leadership or support role over a sustained period of time at the local, state and/or national level(s). 

This award recognizes an individual who has worked toward social justice and health equity for underserved and disadvantaged populations.

This award recognizes an individual who has made an outstanding contribution of major significance to public health during the preceding calendar year and is actively involved in public health in Illinois.  Nomination information should indicate that the individual’s efforts have gone beyond what is normally expected and are exemplary of dedication to the advancement of health of the public. 

This award recognizes an individual or group of public health students who make an outstanding contribution of major significance to the public health movement through community service. 

This award is given to a current agency member who has demonstrated outstanding leadership and service to the Illinois Public Health Association and public health over a period of years.

This award is given to a current individual member who has demonstrated outstanding leadership and service to the Illinois Public Health Association and public health over a period of years.
