The Illinois Public Health Association, through the attached Request for Applications, seeks interested community providers (community-based organizations, local health departments, federally qualified health centers, and other non-profits) to deliver Alzheimer's disease and related dementia (ADRD) early detection and awareness in their communities, with a particular focus on high-risk and underserved populations. The term of the grant will be May 1, 2022 to June 30, 2022, with an expected one-year renewal from July 1, 2022 to June 30, 2023. 

The deadline to apply is 5:00pm CDT on May 9, 2022.
